Decision

Vias stay off SMD pads narrower than 2 mm (--keep-vias-off-pads, any net, net-class clearance), exposed pads excepted

Alternatives

Allow same-net vias in pads (the default); a separate via-in-pad cost

Reason

Plane targets made in-pad vias the cheapest plane connection (about 100 on one board, 0.5 mm vias in 0.25 mm pins), which needs filled and capped vias and spoils fine-pitch lands. 2 mm keeps 0402/0603, QFN pins, crystals and test pads clear and lets exposed pads take thermal vias
